Registration Forms

If you just signed up online for Aspenauts, Powder Pandas, SnoBandits, SnoWarriors, All Mountain, Cross-Country 1-2, Cross-Country 3-4, Spring Gulch Kinder, Nordwarriors or another Base Camp Recreational Program, you're done! There is no need to download and fill out the forms below. Thank you!

Instructions for Devo and Team AVSC only! If you are signing up for a Devo or Team program you must download these forms, fill them out, sign them and fax them to AVSC.

Devo and Team Registration is not complete until all forms have been returned to the AVSC Clubhouse either in person, by mail or by fax.

Individual Forms for Download:

1. PARTICIPANT EXPECTATIONS download
Signature Required for Team AVSC; all parents and participants must read this in order to understand our policies and codes of conduct. If required print, sign, and mail (or fax).

2. PHYSICIAN'S RELEASE download
Signature Required for Devo and Team AVSC; not required for other programs. If required print, sign, and mail (or fax).

3. PERMISSION TO TREAT download
Signature Required for Devo and Team AVSC; not required for other programs. If required print, sign, and mail (or fax).

4. PARENT WORK DEPOSIT FORM download
You may wish to print this out now so you have it on hand during the season.

5. ATHLETE TRAVEL ACCOUNT FORM download
If your child intends to travel with AVSC, you must open a travel account for them. Please download this form to open your child's account.
